They will ensure you’re not only ready to return to your normal daily routine in the comfort of your own home, but will also actively assist you with everyday tasks – such as cleaning, cooking, grocery shopping and attending appointments.

Everyone is different, so your after hospital care plan will be unique and tailored to meet the challenges of your specific health conditions, recovery requirements, daily habits and lifestyle goals. Our Care & Support workers are also selected with your direct input, to match your personality.

Generally speaking, after hospital care is divided into two main areas. The first focus is assisting you with those day to day tasks you may need a little extra help completing, as well as getting you back on track to help manage them yourself as much as possible. The second is focused on your overall wellbeing as a person.

Escort you home

A carer can be with you from the very moment you are discharged from hospital. They can help you with your belongings, transport you safely home and then once home they can help to settle you in.

Help with discharge instructions

Every patient is given discharge instructions and information when leaving hospital. This can be a lot of information to follow and remember. Your dedicated carer can help to remind you about your post hospital care.

Follow up appointments

In many cases you will need some form of follow up care at the hospital. This may be for a routine x-ray or to see your specialist. Your carer can go with you to this appointment and offer much needed moral support.


Prescription and medication

A carer can help to remind you when you need to take your medication. They can also collect any medication that you may need from the pharmacy and help you with repeat prescriptions.

Personal care

You may need a little extra help with personal care needs once you return from hospital. This is especially true if you have had surgery or if you need an extended period of rehabilitation. Live in carers can assist you with all personal care requirements.

Shopping

You may still feel a little weak or have reduced mobility following your hospital stay. A carer can do your shopping and unpack it all for you as well. If preferred they can help you to organise an online delivery.

How long can I receive after hospital care?

Once again it all depends on your specific circumstances and personal preferences. Some of our customers require daily, overnight, or 24 hour care for several months following their return home. Others only require intensive assistance for the first week following discharge, and just a few hours per week afterwards, and may only need minimal help with cleaning or shopping for a short time.

The important thing to know is the care is available as long as you need it to get back on your feet again and remain in your own home!
