  • Strengths-based: The model focuses on an individual’s strengths and abilities, rather than their deficits. This helps individuals to build on their strengths and live their lives to the fullest.
  • Outcomes-oriented: The model focuses on the outcomes that individuals want to achieve, such as being able to live independently, participate in their community, and maintain their relationships.
  • Early intervention and prevention: The model aims to identify and address individuals’ needs as early as possible, to prevent them from needing more intensive support in the future.
  • Co-production: The model encourages individuals, families, and professionals to work together to develop and deliver care and support.

The Connected Lives model is based on the Care Act 2014, which is the legislation that governs adult social care in England. The Care Act emphasizes the importance of individual choice and control, and the Connected Lives model is designed to put these principles into practice.
