Care Co-ordinator
About the Role:
We are seeking a compassionate, organised, and proactive Care Coordinator to join our dedicated team. In this vital role, you will be responsible for managing and coordinating high-quality care services to ensure our customers receive person-centred support that promotes independence, dignity, and well-being.
This is an office based job in Sutton, Surrey.
Key Responsibilities:
- Coordinate and schedule care visits in line with customer care plans, ensuring continuity and consistency of care.
- Act as the main point of contact for care staff and customers regarding rota changes, customer updates, and day-to-day support.
- Liaise closely with care workers, customers, families, and other professionals to ensure smooth delivery of services.
- Monitor and review care plans, conducting regular quality assurance visits and spot checks.
- Support with the recruitment, onboarding, and training of care staff.
- Participate in the on-call rota to provide out-of-hours support as needed.
- Maintain accurate records and ensure compliance with CQC regulations and company policies.
About You:
- Previous experience (preferable but not essential) in a care coordination or scheduling role, ideally within domiciliary care or a similar environment
- Excellent organisational and time management skills.
- Strong communication and interpersonal abilities.
- A genuine passion for delivering high-quality care.
- Proficiency in using care management systems and Microsoft Office.
- Ability to work under pressure and adapt to a fast-paced environment.

Care Assistant
You would be responsible for meeting the support requirements of our customers, in a way that respects the individual and promotes their independence. You would be expected to travel to the customer’s home and assist them with daily tasks as agreed between the customer and Caremark.
The type of support that you would be providing is no different to what would reasonably be given by a member of the person’s family. For instance, you may be providing help with bathing and dressing or domestic support.
Principal Responsibilities
Your role would include helping customers with tasks such as:
- Help with washing & dressing
- Shopping & meal preparation
- Prompting with medication
- Support with daily routines
- Respite services
- Socialising and leisure activities
- Helping customers access the community
